Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Case Management department! The Team Leader Birthing Families Navigator provides direct services to clients as well as supervises a team of Birthing Family Navigators. They work with medical providers and care teams as well as leverage reporting to identify potential clients and integrate Birthing Family support into the Erie sites where their team members are based. They partner with clients to complete assessments, care plans, make referrals, and deliver supportive services based on client needs. The Team Leader Birthing Families Navigator role includes outreach, advocacy, assessments, home visitation as needed, and documentation in funder data management system and electronic health record.

- Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Birthing Families Navigator Role:
- Provides direct services including coordination of primary care (prenatal, post-partum, and/or pediatric care) and social services, outreach, face-to-face follow ups, risk assessments completion, referrals, home visitation as needed, and track clients’ appointment failures.
- Collaborates with fellow Birthing Families Navigators to provide constant floor coverage so as to assure productive presence of a Navigator throughout the day.
- Maintains quality of the cases by achieving Erie’s and grantor’s programs goals
- Completes and reviews monthly reports
- Collects participant’s surveys as required by grantors and returns them to the designated point of collection within the department
- Develop orientation plan, and evaluates direct reports in collaboration with Manager of Quality Assurance and Case Management Programs
- Conducts bi-weekly Quality assurance to measure performance and 1:1 meeting with direct reports
- Provides guidance and support to direct reports regularly through 1:1 supervisory meeting, case reviews and team meetings
Education
Required:
- BA required in social work, child development, community health, or a related field
Required:
- Three (3) years of work experience in health, social or community services required
- Interest in supervising and motivated to learn people management
- Experience in a community health center or health care setting preferred
- Case Management or similar program experience preferred
- Any supervision experience a plus
Required
- Bilingual fluency (English/Spanish) is required depending on the Erie site and/or population served. (May be required to assist or work with non-English/Spanish-only speaking patients and/or guests.)
- Intermediate Computer Skills: Intermediate proficiency in Microsoft Office required.
- Time management skills required.
- Requires knowledge and exceptional ability with written and oral communication skills and working with the public.
- Illinois driver’s license and automobile insurance required.
- Travel between sites (when necessary).
- Experience with electronic health records and funder data management documentation preferred.
