Position: Physical Therapist
Location: Kingwood, WV 26537
Duration: 3 Month contract to start, potential for extension and/or conversion to FTE
Rate: $55.00/Hr
Start Date: ASAP
Description:
Travel Physical Therapist, Home Health/Hospice
Weekly Hour Guarantee: 40
Productivity Expectations (points): 30
Coverage Areas: Dallas service area
Shift schedule (days, start & end times): Mon-Fri 8-4:30
On Call/Weekend Requirement: No
Will Mileage be reimbursed between visits, from first patient to last patient (Y/N)? Y
Responsibilities:
Responsible for seeing 30 points a week
Providing patient care in their homes
Home health experience required Active WV PT licensure
Qualifications/experience:
Home health Experience (at least 2 years preferred)
HCHB experience/ EMR Experience
Experience working in high volume agencies
Experience working with high acuity patients
Type of certification required: Licensed physical therapist
Dress code/scrub color: Any
Min Education: Master’s Degree
