We're seeking a compassionate and experienced Registered Nurse (RN) Coordinator to join a busy outpatient team. In this role, you'll coordinate patient care, collaborate with providers and clinical staff, and help ensure patients receive high-quality, seamless care throughout their healthcare journey.
Responsibilities
- Coordinate patient care plans across providers, departments, and ancillary services.
- Perform clinical triage via phone, patient portal (EMR), and in person.
- Review laboratory, imaging, and diagnostic results, escalating concerns as appropriate.
- Prepare patients for procedures and ensure all pre-operative requirements are completed.
- Educate patients on treatment plans, medications, and follow-up care.
- Coordinate referrals, prior authorizations, and specialty appointments.
- Accurately document all patient interactions in the electronic medical record (Epic).
- Serve as the primary point of contact for patients, families, providers, and the care team.
- Monitor patient progress and identify opportunities to close gaps in care.
- Support quality improvement initiatives and departmental goals.
Qualifications
- Active Registered Nurse (RN) license required.
- 2–3 years of clinical RN experience required.
- 3–5 years of nursing experience preferred.
- Family Medicine or outpatient clinic experience strongly preferred.
- Care coordination or case management experience preferred.
- Experience with clinical triage and patient assessment.
- Proficiency with Epic or a comparable electronic medical record system.
- Strong patient education, communication, and organizational skills.
- Ability to manage multiple priorities in a fast-paced, team-oriented environment.
