We're currently partnered with a growing Home Health organization looking for RNs in the Santa Cruz County area.
This role is pay-per-visit which directly allows for higher earning potential and more autonomy. This role offers PPV rates $90-95/point (plus mileage) with an admin rate of $65/hr.

Key Responsibilities
- Maintain 30 points per week.
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications, treatments, and therapies as prescribed by the physician.
- Monitor patients' conditions and report any changes or concerns
- Educate patients and their families on disease management, medication adherence, and self-care techniques.
- Collaborate with other healthcare professionals, including therapists, social workers, and home health aides, to ensure holistic care delivery.
- Maintain accurate and timely documentation of patient assessments, interventions, and outcomes in electronic health records.
- Current Registered Nurse (RN) license in the state of California
- Reliable and insured vehicle and valid driver's license.
- Minimum of 1 year of clinical nursing experience, preferably in home health or a related setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health record documentation.
