Licensed Practical Nurse (LPN) – Home Health
Location: Virginia Beach / Norfolk / Chesapeake/ Portsmouth/ Suffolk, VA
Schedule: Full-Time or Part-Time

Responsibilities:
- Provide skilled nursing care to patients in their homes under the supervision of an RN.
- Monitor patient health status and report changes promptly.
- Administer medications and treatments as prescribed.
- Collaborate with the care team to ensure coordinated and effective patient care.
- Educate patients and families on health maintenance and disease prevention.
- Timely completion of required documentation
Qualifications:
- **Current LPN license in the state of Virginia.**
- **Minimum 1–2 years of home health nursing experience** IMPORTANT
- Knowledge of Medicare and ACHC standards is a plus.
- Strong communication, organization, and clinical assessment skills.
- Experience with and understanding of EMR systems
- Compassionate, reliable, and dedicated to patient-centered care.
- Wound Care experience
- Lab test collection skills
