Eprine Home Care, a licensed home care agency, is seeking an experienced Intake Specialist to support onboarding of potential referrals.
Key Responsibilities
· Manage and onboard incoming patient referrals efficiently
· Collaborate with agency marketers and internal team members
· Follow agency systems, workflows, and protocols
· Communicate effectively with patients, families, and partners
· In person visits for all start of cares and monitoring
· Ability to attend semiannual visits
· Significant travel in the community
Qualifications
· Experience working with MLTC, CHAA, PACE in a licensed home care agency preferred (training available for the right candidate)
· Strong time management and organizational skills
· Excellent telephone etiquette and communication skills
· Ability to work both independently and as part of a team
· High attention to detail with strong prioritization abilities
· Understanding of MLTC processes
· Proficiency in Microsoft Office (especially Excel)
· Experience with EHR systems, preferably HHA Exchange
· BA Degree in Health and Human Services
· Ability to travel in community
Pay: $30.00 per hour
