Part-Time Employment - Sunday 8am-4pm REQUIRED
Pay Rate: $18.00 hourly
A Wellness Companion will provide companion services and non-medical assistance to help the participant thrive safely, comfortably, and independently in their own home. This role focuses on supporting daily structure, routine, and overall well-being through consistent presence, engagement, and reassurance.
ESSENTIAL JOB FUNCTIONS
- Follow the participant’s person-centered Health and Wellness Plan
- Support the participant in maintaining their daily schedule and routine
- Provide reminders and cueing for activities of daily living as outlined in the Health and Wellness Plan
- Provide medication reminders as outlined in the Health and Wellness Plan
- Encourage use of language assistive devices as appropriate
- Serve and participate in meal planning and encourage adequate hydration
- Promote and participate in physical activity appropriate to the participant’s abilities and preferences
- Support participant safety and awareness when outside the home and in the community.
- Encourage participation in community activities that the participant desires including faith community attendance, indoor and outdoor walks, shopping, and meals
- Arrange rides and accompany the participant utilizing ESLC Transportation, Uber, Lyft, and other community transportation services
- Support the participant in maintaining a tidy home environment
- Complete and submit a detailed note and checklist at the end of each shift
- Consistently uphold the participant’s dignity and treat all individuals with respect
Requirements
QUALIFICATIONS:
- High School Diploma or GED.
- Ability to work with people living with dementia and their caregivers with empathy and professionalism.
- Detail-oriented and thorough: the ability to independently manage the details of work and complete tasks thoroughly.
- A dedicated, energetic, and passionate individual with a strong work ethic
- Ability to observe changes and communicate concerns clearly to the care team
- Strong interpersonal skills with the ability to build rapport and provide supportive, compassionate companionship
- Ability to follow Medicare GUIDE Program person centered plans, routines, and written instructions consistently
- Strong organizational skills and ability to complete detailed, timely documentation
