Home Based Case Manager
Under the agency’s policies and professional requirements the Home Based Casemanager Provision of structured, goal-oriented, time-limited therapy in the natural environment of families who need assistance recovering from physical, sexual, emotional abuse, and neglect. Other issues, including substance abuse, mental illness, personality/behavior disorder, developmental disability, dysfunctional family or origin, and current family dysfunction, may be addressed in the course of treating the abuse/neglect.
**Home- Based Case Managers requires a Bachelor's Degree in psychology, or related field.
Major Responsibilities and Related Tasks:
A. Complete initial contact to referred families.
B. Complete an assessment with the family in an effort to develop appropriate/necessary recommendations for future success. '
C. Provide necessary resource and support system development.
D. Provide family counseling/HBT one to two times weekly.
E. Provide crisis intervention as necessary.
F. Participate in case conferences, school conferences and court appearances with the family as requested.
G. Provide family advocacy as needed.
H. Participate in ongoing assessment and evaluation of the family’s goals.

Knowledge and Skills
Activities are governed through a professional code of ethics and rules of confidentiality. A thorough knowledge of social work practice is expected with emphasis in home-based service delivery. Effective skills should be demonstrated in dealing with families involved in abuse/neglect situations as well as those involved in juvenile delinquency issues. A thorough knowledge of community resources is expected, but can be learned on the job.

The position requires a Bachelor’s Degree in social work, Psychology, or related field from an accredited college or university. Home-based service delivery experience is a plus. Also experience working with Indiana Department of Child Services.
