Per Diem RN. Monthly Follow-Up Visits

Pay: $40 to $50 per visit
Epic Nursing Services is seeking a compassionate and dependable RN to complete monthly follow-up visits for our AFC and GAFC members throughout Massachusetts. This is a per diem position that offers flexibility and is ideal for a nurse looking to supplement their income while making a meaningful impact in the community.
Responsibilities:
- Conduct monthly follow-up visits with members
- Monitor and document changes in health condition
- Follow up with PCPs, managed care plans, and other providers as needed
- Help coordinate and confirm necessary appointments
- Maintain clear, accurate, and timely documentation
- Communicate effectively with agency staff to support continuity and quality of care
Qualifications:
- Graduate of an accredited School of Nursing
- Active RN or LPN license in Massachusetts
- Minimum 2 years of experience working with elderly or disabled individuals
- Ability to travel throughout Massachusetts with reliable transportation
- Strong organizational, communication, and problem-solving skills
- Proficient in Microsoft Word, Excel, and Outlook
- Ability to work collaboratively with staff and leadership
- Current CPR certification required
- Must pass a criminal background check
