We are looking for a compassionate, organized, and proactive Social Services Assistant to support our residents across two of our premier communities: Lighthouse at Chico and Vacaville Senior Living. In this role, you will split your time between both facilities, acting as a vital bridge for our residents and their families as they navigate the complexities of healthcare benefits and long-term care placement.

Key Responsibilities
Split-Site Support: Divide your work week between our Chico and Vacaville facilities, ensuring consistent social services presence and support at both locations.
Benefits Advocacy: Actively assist residents and families with the application, documentation, and redetermination processes for Medi-Cal and Social Security.
Case Management Assistance: Support the Social Services Director in conducting initial intake assessments, developing care plans, and coordinating discharge planning.
Resident Advocacy: Serve as a trusted point of contact for resident rights, helping resolve concerns and connecting them with community-based resources.
Documentation: Maintain accurate, confidential, and timely social services records in compliance with state and federal regulations.
What We Are Looking For
Experience: Previous experience working in a skilled nursing, assisted living, or long-term care setting is highly preferred.
System Knowledge: Proven experience navigating Medi-Cal eligibility, Social Security benefits, and local county assistance programs is required.
Reliable Commute: Ability and willingness to split time between Chico and Vacaville (reliable transportation and a valid driver's license are a must).
Communication Style: Excellent interpersonal skills with a warm, empathetic approach to senior care and family communication.
Organization: Strong multitasking skills to manage a split caseload across two distinct locations effectively.
