Pay: $41,000 - $46,000 Annually

Responsibilities

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
  • Support and closely coordinate mental health care with the patient’s primary care provider and other treating mental health providers.
  • Identify gaps in services and intervene to ensure that the individual receives the appropriate care.
  • Continually monitor and assess patients for common mental and substance abuse disorders.
  • Provide patient education about common mental and substance abuse disorders.
  • Assistance in coordination with significant others to help in the treatment planning process whenever possible.
  • Assistance in coordination with and follow-up for medical services as identified in the treatment plan.
  • Facilitation of referrals to mental health services as identified by a client’s comprehensive assessment or treatment plan.
  • Assistance with referrals to economic assistance, social services, housing resources, and prenatal care according to the client’s needs
  • Support psychotropic medication management prescribed by PCPs, SAP telemedicine, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g, CBT, DBT, IOP, Residential) as clinically indicated.
  • Attend and participate in our clinical supervision team and communicate resulting treatment recommendations to the patient’s clinical case manager and primary care physician as needed.
  • Facilitate patient engagement, attendance and follow-up sessions.
  • Track patient follow-up and clinical outcomes measures.
  • Document patient progress and treatment recommendations in our EMR so that they can be easily shared with the full clinical team, the consulting psychiatrist, psychologist, and other treating providers.
  • Help to complete relapse prevention plans and continuing care plans with clients and primary case manager.

  • Establishes and maintains professional and positive relationships with co-workers.
  • Complies with Meridian Behavioral Health policies, procedures, and code of conduct.
  • Complies with DHS 245G, state, and federal policies, procedures, rules, and regulations.
  • Uses desktop and/or laptop computer and possesses knowledge and experience with Windows Microsoft Office Suite programs and experience with EMR (Procentive).
  • Participates in employee training and orientation.
  • Provides back-up and training to co-workers.
  • Maintains punctual and regular attendance.
  • Maintains required licenses, certifications, and credentials as required of the position.

Skills & Qualifications

Required:
A treatment coordinator must receive at least one hour of supervision regarding individual service delivery from an alcohol and drug counselor each month by a clinical supervisor. An individual is qualified to provide treatment coordination if the individual s skilled in the process of identifying and assessing a wide range of client needs:
  • Is knowledgeable about local community resources and how to apply those resources for the benefit of the client.
  • Has successfully completed 30 hours of classroom instruction on care coordination for an individual substance use disorder.
  • Has at least 2,000 hours of supervised experience working with individuals with substance use disorders.
Has Either:
  • A bachelor’s degree in one of the behavioral sciences or related fields, or
  • Current certification as an alcohol and drug counselor, level I, by the Upper Midwest Indian Council on Addictive Disorders, and
Preferred:
  • Strong verbal skills and ability to communicate abstract concepts in a simple format.
  • Ability to communicate with a variety of people from diverse cultures, socioeconomic and educational backgrounds.
  • Flexibility to meet a variety of changing priorities.
  • Computer skills to draft emails, memos, and reporting.
Physical Requirement
  • Extended periods of sitting, 80% of the time.
  • Occasionally reaching for and lifting client charts, 5lbs 10% of the time.
  • Heavy data entry on computer, 25% of the time.
  • Frequent handling of paperwork, binders, and writing.
  • Sitting 6-8 hours per day
