Description
As an RN Case Manager at JBSA Lackland, you will provide specialized nursing care and case management services for active-duty service members, dependents, and other beneficiaries within the military health system. You will coordinate comprehensive assessments, develop and implement individualized care plans, collaborate with the multidisciplinary team, and ensure continuity of care in alignment with the mission of the 59th Medical Wing and the health readiness of the force.
Key Responsibilities
Conduct in‐depth assessments of medical, psychosocial, functional, and deployment/readiness related needs for service members and beneficiaries.
Develop, implement, monitor, and revise individualized case management plans in coordination with providers, family support services, mental health, TRICARE/benefits, and other stakeholders.
Serve as the primary point of contact for case management services: referrals, transitions of care, discharge planning, liaison with civilian/contracted providers when appropriate.
Monitor utilization of health services, track outcomes, identify barriers to care, and work to optimize resource use and care delivery.
Provide education, advocacy, and support to patients, families, and the care team — especially around readiness, deployment, chronic condition management, and health system navigation.
Maintain accurate, timely documentation in the military health record system; ensure compliance with DoD/AF/59th Medical Wing policies, regulatory standards, and quality improvement initiatives.
Participate in multidisciplinary care team meetings, case conferences, readiness briefings, and quality assurance/performance improvement activities.
Assist in crisis intervention, readiness issues, and support for unique military population needs (PCS, deployment, family transitions).
Maintain patient confidentiality, security‐clearance compliance, and professionalism consistent with military environment requirements.
Skills, Knowledge and Expertise
Current, unrestricted Registered Nurse (RN) license in Texas or eligible to obtain Texas licensure, or part of Nurse Licensure Compact (if applicable).
Bachelor of Science in Nursing (BSN) Required.
Minimum of 2–5 years of professional nursing experience; prior case management, care coordination, or clinical team leadership preferred.
Experience in a military health system or familiarity with TRICARE, DEERS, service member readiness, and military family issues is highly desirable.
Strong clinical assessment, critical thinking, and care‐planning skills.
Excellent communication, collaboration, and documentation skills.
Ability to work effectively in a fast‐paced, mission-driven, multi‐disciplinary environment.
Proficiency with electronic health records (EHRs) and case management software.
Willingness to comply with base access, security clearance requirements, and possible shift flexibility (including readiness or deployment support).
