What are the responsibilities and job description for the Case Manager position at EnvisionAge Incorporated?

The Case Manager at JBSA Lackland supports active-duty service members, their families, dependents, and other beneficiaries by coordinating care, resources, and services across the 59th Medical Wing. This role involves comprehensive assessments, development and implementation of individual care plans, liaising with multi-disciplinary teams, and ensuring continuity of care in a military health system environment.

Key Responsibilities

Conduct thorough assessments of medical, psychosocial, and service-related needs for active-duty members, families, or beneficiaries.

Develop, implement, monitor, and revise individualized case/care management plans aligned with military health system goals and WDUs/training schedules.

Coordinate services across clinics, specialty care, mental health, family support, and external civilian providers as needed.

Advocate for the patient/family, ensuring access to required resources, benefit programs, and military-specific support (e.g., TRICARE, DEERS updates).

Collaborate with primary care providers, specialty physicians, nurses, and other care team members to facilitate timely appointment scheduling, follow-up, and discharge planning.

Provide education and support regarding medical conditions, service transitions, deployment/re-deployment health issues, and family readiness.

Monitor utilization of services, track outcomes, and identify barriers or gaps in care within the military health system context.

Maintain detailed, compliant case documentation in the military electronic health records system, meeting regulatory and quality assurance standards.

Participate in quality assurance, utilization review, and performance improvement initiatives specific to military health services.

Ensure confidentiality and compliance with DoD, Air Force, and 59th Medical Wing policies regarding patient privacy, records, and security.

Skills, Knowledge and Expertise

Bachelor's degree in Social Work, Nursing (RN), Psychology, Human Services, or related field. A master's degree preferred.

Certified Case Manager (CCM) or equivalent preferred OR willingness to obtain.

Minimum 2-5 years of case management experience or equivalent in a healthcare or military/DoD environment.

Strong knowledge of military health system processes, TRICARE, DEERS, and family readiness support is a plus.

Excellent communication, organizational, and documentation skills.

Ability to work effectively in a fast-paced, mission-driven environment with active-duty service members and their families.

High degree of cultural sensitivity and adaptability to the unique needs of military life (deployments, PCS moves, etc.).

Proficiency with electronic medical records (EMR) and case management software; ability to learn military systems.

Security clearance or ability to obtain/maintain one may be required depending on position status (civilian/contractor).
