Key Responsibilities
- :Home Visits: Conduct home visits to assess and create personalized care plans for clients and their caregivers
- .Training & Support: Train and support caregivers, ensuring they can meet their loved ones' needs
- .Reporting & Coordination: Monitor and report on clients’ health, coordinating with their primary care physicians, community resources
- .Ongoing Care: Facilitate smooth transitions and ongoing care for clients
- .Compliance: Maintain compliance with all relevant regulations and standards
- .Community Awareness: Participate in community events to spread awareness and enroll new caregivers
- .Role Model: Embody Entyre Care’s mission, values, and culture, acting as a role model for the team

What You Will Brin

- g.Outstanding proficiency in common software and web-based applications including Microsoft Excel, Word, PowerPoint, and Team
- s.Master's degree in social work or a related field preferre
- d.Experience working in Structure Family Caregiving (SFC) or a similar setting preferre
- d.Certifications in gerontology or case management preferre
