Position Overview
Key Responsibilities:
- Home visits: Conduct home visits to assess and create personalized care plans for clients and their caregivers.
- Training & Support: Train and support caregivers, ensuring they can meet their loved ones' needs.
- Reporting & Coordination: Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more.
- Ongoing care: Facilitate smooth transitions and ongoing care for clients.
- Compliance: Maintain compliance with all relevant regulations and standards.
- Community awareness: Participate in community events to spread awareness and enroll new caregivers.
- Role Model: Embody Entyre Care’s mission, values, and culture, acting as a role model for the team.
What You Will Bring:
- RN license required.
- Bachelor’s degree appreciated not required; preferably in social work, human services, or a related field.
- Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
- At least one (1) year of experience in-home healthcare.
- Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
- Experience supervising caregivers preferred
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).
