The Targeted Case Manager will work closely with clients, their families, and other healthcare providers to ensure comprehensive care and support. Additionally, the Targeted Case Manager will advocate for clients and assist them in accessing necessary services and support systems.

Qualifications

- Bachelor's degree in Social Work, Psychology, Human Services, or a related field required per state regulations
