Position Summary:
The Case Management Supervisor (RN) works as a leader within the Care Management Team supporting the coordination of care and services for patients in collaboration with physicians, the full care team, payors, and patients/families while facilitating patient centric, high-quality care delivery. This role leads a team of nurses, CMAs, regulatory clerks and others to manage patient throughput, ensuring compliance with state and federal regulations. Key administrative duties include: orienting, training, supervising and evaluating staff, preparing/finalizing work schedules, managing timecards, provides approval for select budgetary items, and performs chart audits for RNCCs, Regulatory Clerks, CMAs and other staff as appropriate. This role provides consultation organization-wide regarding difficult cases/innovative strategies related to care coordination; coordinates execution of Enloe Health’s’ collaborative Case Management program, participates and takes on leadership responsibilities in committees, the community, and professional organizations; provides ongoing evaluation and assessment of productivity and other metrics, and performs other duties as required.
Additionally, this role focuses on guiding teams in reducing unnecessary length of stay, facilitating standard operating procedures for safe transitions, oversight of daily operations of the case management department, and provides leadership to staff to ensure efficient utilization of hospital resources, quality patient care, and effective, compliant discharge planning. Builds and sustains strong collaborative relationships with organizational leaders, community partners, physicians, and the interdisciplinary team to promote integrated, patient‑centered care delivery. Must be flexible and able to adapt to shifting priorities. Demonstrates high emotional intelligence, including self‑awareness, empathy, adaptability, and the ability to navigate complex interpersonal dynamics with professionalism and composure. Excellent verbal and written communication and motivational skills with ability to communicate within all levels of the organization. Understands integrated care team concepts and approaches situations holistically. Works collaboratively with the Social Work Supervisor in a co-management model. Behavior should be reflective of Enloe Health core values. Must be able to fulfill the essential functions of the position.

Minimum:
EDUCATION / TRAINING / EXPERIENCE:
- Three years of case management experience
- Experience utilizing computers and Microsoft Office programs, Midas, Epic or other EHR
- Experience in developing and adapting new care processes and programs
- Knowledge of local, state and federal resources, confidentiality and risk/malpractice issues within healthcare settings
- Master’s degree in Nursing, Leadership or other related area
LICENSES / CERTIFICATIONS:
- Current RN licensure with the California board of registered nursing
- Current BLS certification
- Certified Case Manager (CCM or Accredited Case Manager (ACM) certification
