Job description:
At Enlightened Recovery, we provide exceptional medical and clinical services. We are seeking a full-time Primary Counselor. This role is responsible for performing client care activities as directed by the Clinical Director. This position will provide group therapy and collaborates with the Treatment Team in the formulation of the Master Treatment Plan, and assists in coordinating discharge planning.
- Actively participates in Multidisciplinary Treatment Plan meetings.
- Shares pertinent information with the team as obtained from client’s family, or others as indicated throughout the program.
- Evaluates the impact of therapeutic interventions upon the therapeutic milieu.
- Performs case management tasks with client/family. Caseload will include co-occurring clients.
- Assesses clients using ASAM and references DSM to determine need and appropriate level of care, continued need for services and transfer/discharge.
- Collaborates with client to develop a treatment plan and revise as needed.
- Creates transfer/discharge plan as required.
