Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
• Administer medications and perform specialized care procedures such as catheter care, Hoyer lift transfers, and hospice support under supervision or as trained.
• Support clients with memory care needs by implementing personalized routines for Alzheimer’s and dementia patients to ensure safety and comfort.
• Maintain detailed medical documentation in compliance with HIPAA regulations to ensure confidentiality and accuracy of patient records.
• Prepare nutritious meals aligned with dietary restrictions and assist with meal planning and preparation to promote health.
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to support overall well-being.
• Collaborate with healthcare teams to communicate changes in patient conditions and update medical records promptly.
• Use medical terminology accurately when recording observations or communicating with healthcare professionals.
• Employ proper infection control practices during patient care activities to uphold safety standards.

Experience

• Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred.
• Demonstrated ability to work effectively with individuals who have developmental disabilities or require specialized dementia or Alzheimer’s care.
• Familiarity with medical procedures such as vital signs measurement, medication administration, catheter care, and hospice support.
• Experience utilizing EMR/EHR systems for documentation and patient record management.
• Strong understanding of patient observation techniques and the ability to recognize changes in health status promptly.
• Knowledge of medical terminology relevant to home health caregiving roles.
• Certifications such as CPR (Cardiopulmonary Resuscitation) are required; additional certifications like Hoyer lift training or hospice care experience are advantageous.
• Long-term care experience is beneficial for providing consistent, high-quality support over extended periods. Pay: $12.00 - $13.00 per hour
