Clinical Liaison Home Health Primary Location: Fort Myers, Florida Req ID: 2026-148423 Requisition Type: Sales Patient Coverage Area: Lee County Overview As a Clinical Liaison (Care Transitions Coordinator), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services. Responsibilities Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care. Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities Qualifications Education and experience, essential Must be a graduate of an approved school of nursing, therapy or social work. Must be licensed in the state where they currently practice. Must have two (2) years of demonstrated field experience. Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred A Registered Nurse RN, Physical Therapist PT or Licensed Medical Social Worker LMSW is preferred. Three (3) years of field experience is preferred. Previous experience in home health or healthcare sales is preferred.
