Role Description The Clinical Care Transition Coordinator is a full-time, hybrid role based in Port Charlotte, FL, with opportunities for some work from home. In this role, the coordinator supports patients transitioning into home health or hospice services, ensuring care plans are clearly communicated and implemented across settings. Daily responsibilities include collaborating with referral sources and care teams, reviewing clinical documentation, coordinating case management activities, and helping patients and families understand available services. The coordinator may provide training and education to internal teams and external partners on transition processes, home care options, and program benefits. This role also engages in outreach and relationship-building with healthcare partners to promote Enhabit’s services and support appropriate referrals.
Qualifications
- Candidates should possess strong Communication skills, including clear, compassionate interaction with patients, families, and interdisciplinary teams.
- Candidates should possess Case Management and care coordination skills to support smooth transitions into home health and hospice services.
- Candidates should possess Home Care knowledge, including familiarity with home health and hospice regulations, processes, and best practices.
- Candidates should possess Training and education skills to guide staff, patients, families, and partners through transition protocols and service options.
- Candidates should possess Sales and relationship-building skills to develop and maintain referral sources and promote Enhabit’s clinical programs.
- Relevant clinical background (e.g., RN, LPN, social work, or similar healthcare experience) in care coordination or home health is preferred.
- Ability to work effectively in a hybrid environment, manage time independently, and use electronic health records and standard office software.
