The Clinical Care Transition Coordinator in Lee County, FL, serves as a critical touchpoint for patients transitioning into care. This is a full-time, on-site position based in Fort Myers, FL. Responsibilities include coordinating patient care transitions, communicating with patients and healthcare teams, building relationships with referral sources, and providing education on available services. The role fosters aligned care plans, ensures a smooth onboarding process for patients, and supports our mission of optimizing their healthcare experience.

• Excellent Communication skills to effectively collaborate with patients, families, and healthcare teams
• Experience in Case Management and coordinating care within a healthcare or home care setting
• Knowledge of Home Care practices and guidelines for delivering care in a home-based environment
• Proficiency in Training and educating patients, families, and staff about care plans and services
• Proven ability in Sales and building relationships with referral sources
• Strong organizational and problem-solving abilities
• Active nursing license or equivalent clinical qualification preferred
• Prior experience in home health, hospice, or related fields is an advantage
