Responsibilities
- Deliver direct support to clients by assisting with activities of daily living (ADLs), including personal care, meal preparation, medication reminders, and companionship.
- Observe and monitor patients’ health status, and behavioral changes; document findings accurately in compliance with HIPAA regulations.
- Implement individualized care plans tailored to each client’s needs, including dementia care, Alzheimer’s care, memory care, autism support, and neurodevelopmental or intellectual disabilities.
- Coordinate services by communicating effectively with clients, families, healthcare providers, and community resources to ensure seamless care delivery.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and sanitation in accordance with health standards.
- Assist with transportation needs for appointments or community outings while ensuring client safety during transfers and travel.
- Support clients’ rights by respecting their preferences and promoting independence within the scope of caregiving responsibilities.
Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments.
- Strong patient observation skills to identify changes in condition or behavior promptly.
- Knowledge of patient monitoring techniques
- Familiarity with HIPAA regulations to ensure confidentiality of patient information.
- Experience supporting individuals with dementia, Alzheimer’s disease, autism spectrum disorder (ASD), developmental disabilities, or neurodevelopmental disorders.
- Excellent communication skills for effective client interaction and team collaboration.
- Competency in emergency response procedures such as CPR and first aid certification.
- Ability to adapt caregiving techniques to meet the unique needs of each individual while maintaining professionalism and compassion.
