Overview:

We are seeking a dedicated and knowledgeable Diabetes Educator to join our healthcare team. The ideal candidate will possess a strong background in clinical nutrition and patient education, with a focus on empowering individuals to manage their diabetes effectively. As a Diabetes Educator, you will play a vital role in developing personalized care plans, providing comprehensive nutrition counseling, and collaborating closely with healthcare professionals to optimize patient outcomes. This position offers an opportunity to make a meaningful impact on patients’ lives through education, support, and expert guidance.

Responsibilities:

  • Conduct thorough patient assessments to evaluate individual health status, nutritional needs, and diabetes management goals.
  • Develop and implement tailored care plans that incorporate medical nutrition therapy (MNT), lifestyle modifications, and medication adherence strategies.
  • Provide detailed nutrition counseling and health coaching to support patients in managing their diabetes and related chronic conditions.
  • Utilize motivational interviewing techniques to encourage behavioral changes and foster patient engagement.
  • Monitor patient progress through electronic health records systems, documenting interventions and adjusting plans as necessary.
  • Educate patients on topics such as blood glucose monitoring, insulin administration, self-management, carbohydrate counting, weight management, and dietary restrictions.
  • Stay current with advancements in clinical nutrition, physiology knowledge, and diabetes management protocols to provide evidence-based care.

Qualifications:

  • Bachelor's degree in dietetics, nutrition, or a related field; advanced certifications are highly preferred.
  • Certified Diabetes Educator certificate is required.
  • Proven experience working with individuals managing diabetes or other chronic diseases in hospital or long-term care settings.
  • Strong knowledge of physiology, medical nutrition therapy (MNT), and dietary interventions specific to diabetes management, insulin education and training on diabetic technology (CGMs, glucose meter and insulin pens).
  • Demonstrated expertise in health coaching, motivational interviewing, and client communication skills.
  • Certified Pump Trainer is preferred.
