Responsibilities/essential functions: Must be able to perform the following essential job functions with or without reasonable accommodations.
- Follows the instructions of the registered nurse in providing patient care.
- Bring Love and Joy into the patient's lives and daily activities.
- Provides patient care and housekeeping duties as directed by the aide assignment and plan of care.
- Provide hands-on personal care, assistance in ambulation and transfer, assistance in exercise, provide nutritional support, assist with medications that are ordinarily self-administered, and simple procedures as an extension of therapy or nursing services.
- Perform and record accurate measurements (i.e., vital signs, weights, intake) as instructed by the aide assignment.
- Observe and report issues and/or changes in patient condition to the registered nurse.
- Document's care provided at point of care in the medical record.
- Attends and participates in staff meetings, completes education, and attends education in-services.
- Maintains compliance with Medicare Conditions of Participation, State and Federal laws, Arizona Nurse Practice Act, Joint Commission accreditation standards, and hospice policy and procedures.
- Actively participates as a member of the team and demonstrates the organization mission and values in performance and behavior.
