- Bachelor's degree in theology or related degree.
- Experience providing spiritual, grief, and loss support required.
- Excellent communication, organizational, and interpersonal skills.
Must be able to perform the following essential job functions with or without reasonable accommodations.
- Assess the patient for spiritual needs and implement interventions based on patient goals.
- Completes a comprehensive assessment and plan of care within 5 days of admission.
- Facilitates visits by local clergy, pastoral, or other local faith leaders as requested by patients.
- Educates the patient and family on hospice services and the role of spiritual support..
- May conduct religious services of prayer and worship at the request of patients and families.
- May participate in providing bereavement services.
- Participates in the Quality Assurance Performance Improvement Program.
- Documents at point of care; documentation is accurate and reflects patient decline.
- May provide spiritual support to hospice employees and other community members when applicable.
- Completes education, participates in education in-services, continuing education, seminars, and self-study.
- Maintains compliance with policies, procedures, State and Federal regulations, and accreditation standards.
