TCM Care Coordinator

Encompass Medical Group is currently recruiting for a full-time TCM Referral Coordinator within our Lenexa administrative office.

Working Hours: 8:00am-5:00pm Mon-Fri

Position Summary: The Referral Coordinator is responsible for assisting with Population Management and direct outreach to patients.

Position Responsibilities:

• Responsible for pulling medical records from hospital portals
• Conducts patient outreach for point of care discharge dates
• Offers assistance with copy service
• Handles TCM calls and inpatient ED utilization
• Maintains patient confidentiality
• Other clerical duties as assigned by the supervisor
• Performs other duties as assigned or requested

Job Specifications:

Minimum Education

• High School education or equivalent

Minimum Experience

• GE Centricity experience preferred
• Previous working experience in a medical setting environment required
• Previous referral coordination experience preferred, but not required
• Knowledge of medical field terminology preferred, but not required
• Any equivalent combination of experience and training which provides the required knowledge, skills and abilities

Working Conditions/Physical Demands:

• Regular, climate-controlled office environment

Administrative skills:

• Excellent typing, writing, and spelling skills
• Good phone and voicemail etiquette
• Ability to communicate effectively with office personnel and the public
• Ability to work with minimal supervision

Pay: $16.00 - $17.00 per hour
