POSITION SUMMARY (Used to describe the position for recruiting/ advertisements)

The Care Manager is the point of contact between the client and their care needs. They serve as the advocate, support and navigator, assisting with the needs of the client. 

ESSENTIAL JOB FUNCTIONS | RESPONSIBILITIES, TASKS, DUTIES

  • Provide an initial assessment & ongoing assessments of client as needed/requested
  • Create and executing of Care Plan and/or Framework
  • Oversee external agency caregiver performance and behavior as needed
  • Arrange and have oversight of all in-home help/vendors (caregivers, coordinating with home health, hospice, outside care agencies, etc.)
  • Attend in home medical appointments (PT/OT/RN Home Health Visits)
  • Arrange, oversee, and attend appointments for external help/vendors/medical providers (DME Companies, home projects companies, Estate Planning Attorneys, Financial Planners, physicians, etc.)
  • Act as the emergency contact for clients when appropriate
  • Report suspected or witnessed abuse to proper authorities immediately and accurately
  • Train agency caregivers on site, creating tasks per shift, ensuring they are executed
  • Participate in monthly internal enABLE training and execution
  • Attend weekly internal meetings
  • Act as Family/Client liaison regarding any aspect of care or other need (including accurate and timely reporting to family/responsible parties)
  • Visit every client in case load at least once per month and more often as needed
  • Provide crisis intervention
  • Provide client and family education, guidance, and advocacy as needed
  • Provide family mediation and family meetings
  • Assist with coordination and documentation of medical visits as needed
  • Refer clients to external experts and resources as needed
  • Assess and coordinate social services to combat isolation and improve client wellbeing as needed
  • Assess and coordinate nutrition, groceries, home safety, and other supplies as needed
  • Execute assessments, care plans, frameworks, family questionnaires, and wellbeing checklists as needed for each client
  • Visit client during any hospital or skilled nursing facility stay, as well as after any change in condition, etc., within 24 hours of admission.
  • Perform basic financial planning, long-term planning, and goal execution as appropriate
  • Document notes and visits as well as billable and non-billable hours, mileage and expenses in CareTree daily
  • Perform other office/admin tasks as required by the position and company
  • Collaborate with other care managers and enABLE team on cases as needed to assist, give input, or cover cases as needed
  • Answer phone calls, texts, and emails in a timely manner*, including on nights and weekends as needed, unless on-call coverage is arranged
  • *Timely manner is defined as:
  • Emails: Within 24 hours 
  • Calls and Texts within 1 hour M-F 8am- 6pm 
  • Afterhours: By the next business day
  • Become certified as a Care Manager and joining ALCA


Education and Experience

  • 1 year of experience as a Care Manager or in the Aging Services Field
  • B.A. in Social Work, Psychology, Gerontology or other related fields  
  • Combination of education and experience may be considered

  • Computer skills – proficient in using Microsoft Office programs, typing and POS systems and Apple Products.
  • Language skills – capable of reading and writing the English language in a professional manner
  • Organization skills – maintaining tasks and objects in a logical arrangement so they are easily located
  • Problem-solving skills – discovering, analyzing and fixing issues that need attention
  • Communication – capable of positive communication using verbal, written and body language
  • Concentration – to work with accuracy, with minimal supervision and make appropriate decisions
  • Teamwork – work with diverse individuals/ employees and still accomplish company and personal goals

  • Within 1 year of employment, sit for Care Management certification exam (First attempt to be paid for by enABLE) 
  • Valid and Clean Driving License/Record

  • Must be able to work indoors and outdoors in variable weather conditions and extreme temperatures
  • Must be able to work in a moderately noisy environment with radios, multiple conversations, printers (list if ear protection is provided and required)
  • Must be able to listen, hear and talk in the English language
  • This position is very active and requires employees to frequently stand or sit for long periods of time
  • Regularly required to walk, kneel, stoop crouch, reach, lift, carry, push and pull 
  • Occasionally required to climb or balance, shovel, and operate machinery – such as a forklift or loader
  • Vision abilities are required for driving which include close, distance, color, peripheral, depth perception and ability to adjust focus
  • Must be able to frequently lift a minimum of 50 pounds without assistance and occasionally lift up to 100 pounds with the assistance of another team member or mechanical assistance (such as a hand cart, etc)
  • Listen, see, talk, hear, smell, taste ~ these senses may be necessary with certain job, but not all (be careful not to discriminate – in some cases a simple accommodation may be made)
