PRP (Case Manager)

Job Overview

Duties

• Conduct thorough patient assessments, including medical history review, physiological evaluation, and psychosocial needs analysis.
• Develop individualized care plans that align with clinical guidelines and patient preferences, emphasizing acute care, inpatient, outpatient, and post-acute settings.
• Coordinate with healthcare providers across hospital departments such as ICU, emergency medicine, pediatrics, and behavioral health to ensure optimal patient care.
• Manage electronic health records (EHR) efficiently while maintaining strict adherence to HIPAA regulations to protect patient confidentiality.
• Facilitate discharge planning by connecting patients with community resources, social services, mental health support, addiction counseling, and home health agencies.
• Prioritize tasks effectively to handle multiple cases involving complex medical conditions or disabilities while ensuring timely interventions.
• Communicate clearly with patients and their families to explain treatment options, provide emotional support, and foster understanding of care plans.

Requirements

• Proven hospital experience with exposure to Level I Trauma Centers or Level II trauma centers is highly desirable.
• Strong background in acute care environments such as ICU, emergency medicine, or hospital medicine.
• Experience working with children and individuals with disabilities or substance use disorders is preferred.
• Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management principles.
• Familiarity with electronic health records (EHR) systems and healthcare compliance standards including HIPAA.
• Certification in Basic Life Support (BLS) and CPR is required; additional certifications in addiction counseling or mental health counseling are a plus.
• Excellent client communication skills with the ability to work effectively with diverse populations from various cultural backgrounds.
• Demonstrated ability to prioritize tasks efficiently while managing multiple cases in inpatient or outpatient settings.
• Strong understanding of community resource coordination in health services social work and utilization management practices.

Pay: $25.00 - $35.00 per hour
