Empower's Adult Services Case Coordinators (social workers) work in the community visiting client homes and providing face to face weekly assistance with a variety of needs including medical management, resource referral, housing stabilization and search and PCA surrogacy. Our team of Case Coordinators (social workers) are dedicated to serving persons with physical and cognitive disabilities and helping them to remain living independently.
Case Coordinator Qualifications:
- Bachelor’s degree only
- No licensure required!!
- Experience working with disabilities, either personal or professional
- Ability to travel independently throughout assigned territories in Southeastern Massachusetts
- Have excellent verbal and written communication skills.
