Position Overview:

We are seeking a Work Readiness Case Manager to support adults with disabilities in preparing for meaningful, community-based employment. This role focuses on providing individualized support, managing a participant caseload, and delivering engaging, pre-developed job readiness training in a group setting. If you enjoy building supportive relationships, helping people grow, and presenting interactive learning experiences, this might be the perfect opportunity for you!

 

Schedule: Full-time, Monday through Friday, 8:00 AM – 4:00 PM

 

Key Responsibilities:

  • Manage a caseload of participants receiving work readiness services
  • Conduct intake interviews to assess eligibility and vocational readiness
  • Develop and maintain individualized service plans to support participant goals
  • Provide one-on-one coaching to build confidence, workplace behaviors, and soft skills
  • Present structured, pre-developed job readiness training to small groups
  • Track participant progress and complete accurate case documentation and reports
  • Communicate regularly with internal teams, families, caregivers, and referral sources
  • Collaborate with other departments to align support services with employment pathways
  • Contribute to a supportive, inclusive, and empowering environment for participants

Qualifications:

Required Qualifications:

  • Bachelor’s degree in education, human services, psychology, or related field
  • 1 years of experience working with individuals with disabilities
  • Experience facilitating groups, coaching, or teaching
  • Strong communication (written and verbal) and collaboration skills
  • Excellent multi-tasking, organization, and time management skills
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint)
  • First Aid/CPR certification or willingness to obtain

 

Preferred Qualifications:

  • Familiarity with employment services or disability supports
  • Experience with case management or person-centered planning
  • Bilingual in Spanish is a plus

 

Physical Demands & Work Environment:
The employee is regularly required to talk and hear. The employee frequently is required to use their hands or fingers to handle or feel objects, tools, or controls. The employee is occasionally required to stand, walk, sit, reach with hands and arms, climb or balance, and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus. The noise level in the work environment is usually low.
