We are seeking a compassionate and organized Admission Concierge to join our team! As an Admission Concierge, you will play a critical role in ensuring a seamless transition for new patients and their families, coordinating the admission process, and providing exceptional customer service.
Responsibilities:
- Coordinate the admission process for new patients, including gathering necessary information, completing paperwork, and scheduling appointments
- Develop and maintain relationships with patients, families, and healthcare providers to ensure a smooth transition to our care
- Utilize expertise to identify and resolve issues that may impact the admission process
- Communicate effectively with patients, families, and healthcare providers to address concerns and provide updates
- Maintain accurate and up-to-date records of patient admissions, including demographic information, medical history, and treatment plans
- Collaborate with the hospital and healthcare team to ensure a comprehensive approach to patient care
- Participate in quality improvement initiatives to enhance the patient experience and improve outcomes
- Nursing home Skilled Admission Concierge experience preferred
- Ability to work in a fast-paced environment with high attention to detail and excellent organizational skills
- Exceptional communication and interpersonal skills
- Ability to work effectively with patients, families, and healthcare providers
- Familiarity with electronic medical records and medical terminology
