RN Hospice Case Manager

Trustbridge Hospice, a proud member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Boynton Beach/Delray Beach area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What you'll Do

• As a Hospice RN Case Manager, you will play a central role in coordinating high-quality hospice care while supporting patients and families with empathy and respect.

  • Uphold and actively support the organization’s mission, vision, and values
  • Coordinate comprehensive care for patients and families in collaboration with the primary physician and interdisciplinary care team
  • Identify and assess patient and family needs, ensuring individualized and responsive care
  • Serve as an integral member of the Interdisciplinary Group (IDG), contributing to and following a personalized plan of care
  • Utilize strong clinical assessment, monitoring, intervention, and teaching skills to maximize patient comfort and quality of life
  • Provide compassionate guidance and education to patients and caregivers throughout the hospice journey
  • Prioritize comfort and care as the highest standard in all patient and family interactions
  • Deliver consistently personalized, sincere, and engaging service
  • Ensure patient and family satisfaction by promptly and effectively addressing concerns
  • Maintain the highest level of confidentiality and respect in all aspects of patient and family care

 What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
