What are the responsibilities and job description for the RN Home Care - Stratum LIFE/PACE position at Empath Health?
Stratum LIFE/PACE, a member of Empath Health is currently seeking a Home Care Registered Nurse to join our team.
Provide direct and indirect primary nursing care to assigned participants to include center and home care needs. Works under the direct and indirect supervision of the Home Care Coordinator RN, The Elder Services Clinical Director and Elder Services Medical Director but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible. Supervises both licensed and unlicensed personnel. Directly reports to the Home Care Coordinator RN.
Position Requirements:
- Education and/or Experience: Graduate of a school of professional nursing program required. Bachelor of Science Registered Nursing (BSRN) strongly preferred.
- Licensure:
- Current Florida Registered Nurses License
- Valid CPR certification from the American Red Cross or American Heart Association.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
