Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health RN to provide skilled nursing care for patients in their homes throughout the Fort Myers area (LaBelle and Lehigh Acres).
Schedule: PRN role offering 5-10 visits per week. Potential for full-time in the future if desired!

What You’ll Do
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled nursing services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need
- Graduate of an approved nursing program with an active Florida RN license
- Prefers BSN or MSN degree
- Minimum of 1 year of direct patient care experience
- Home health RN experience with Oasis documentation strongly preferred
- Prefers experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
