Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health PT Physical Therapist to work with us to provide exceptional and individualized care in west Pasco county (New Port Richey, Hudson, Trinity, and surrounding areas)

Locations: Home health visits in west Pasco county (New Port Richey, Hudson, Port Richey, Holiday, Trinity, and surrounding areas)
Visits may also be available in Pinellas, east Pasco, Hillsborough, and/or Hernando counties if preferred!
Schedule: PRN; 5-10 home health visits per week. Required weekend/holiday on-call rotations.
What you'll Do
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled physical therapy services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need
- Active Florida PT license
- Minimum of 1 year experience as a licensed Physical Therapist (PT).
- Home health care experience strongly preferred.
- Prefers experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
- Willingness to participate in weekend and holiday on-call rotations
