Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in Port Charlotte/North Port and surrounding areas.

Locations: Home health visits in Charlotte county (Port Charlotte, North Port, Rotonda West, Englewood, Venice, & surrounding areas)
Schedule: PRN; 5-10 home health visits per week
What You’ll Do
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled occupational therapy services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need
- Active Florida OT license
- Minimum of 1 year experience as a licensed Occupational Therapist (OT).
- Home health care experience strongly preferred.
- Prefers experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
