Home Health OT Occupational Therapist (PRN) – Empath Home Health - West Pasco county

Empath Home Health, a part of Empath Health is growing! We’re expanding our home health services in west Pasco county (New Port Richey, Hudson, Trinity, and surrounding areas) and looking for a passionate PRN Home Health OT Occupational Therapist to join our team.

In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Locations: Home health visits in west Pasco county  (New Port Richey, Hudson, Holiday, Trinity, Odessa, and surrounding areas).

Schedule: PRN; 5-10 home health visits per week.

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
