Empath Home Health, a member of Empath Health is seeking a caring and motivated OT Occupational Therapist PRN Home Health. The Home Health Occupational Therapist will cover the Ocala/The Villages area (choice of Marion, Lake, and/or Sumter counties).

What You'll Do
- Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
- Performs initial and on going assessments to determine level of functioning and patient’s ability to perform activities of daily living, including OASIS assessments at appropriate time points.
- Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.

What You'll Need
- Bachelor Degree in Occupational Therapy.
- Florida licensure in occupational therapy.
- Minimum of one (1) year experience as a licensed Occupational Therapist.
- Home health care experience preferred.
