Medical Director HMC

Hospice of Marion County, a member of Empath Health, is seeking a dedicated Medical Director to provide clinical leadership and ensure the highest standards of palliative and hospice care. In this role, you will guide symptom management, certify hospice eligibility, and collaborate closely with interdisciplinary teams to support patient‑centered plans of care. The Medical Director also plays a key role in dementia‑focused education and serves as the clinical lead for the GUIDE program, helping determine eligibility and shaping individualized care.

What you'll Do:

• Provide expert palliative and hospice medical care, ensuring effective symptom management and alignment with each patient’s goals of care across home and inpatient settings.

• Conduct hospice pre‑election evaluations and counseling, certify eligibility for hospice admission, and complete required recertifications and face‑to‑face visits.

• Collaborate closely with the interdisciplinary team (IDT/IDG) to guide care planning, oversee clinical needs, and support high‑quality, patient‑centered services.

• Participate actively in weekly IDT meetings as Team Medical Director, offering clinical leadership and ensuring care meets regulatory and organizational standards.

• Serve as Medical Director for the GUIDE program, determining patient eligibility and contributing to individualized plans of care.

• Provide clinical expertise for dementia‑focused education initiatives, supporting best practices and organization‑wide competency in dementia care.

• A medical degree and Florida Licensure are required.
• Board certified in Hospice and Palliative Medicine preferred.
• Demonstrated work experience in health care related industries or areas which are translatable to Hospice. Experienced in Hospice industry preferred.
• Willing to prepare for and meet the requirements for eligibility for the American Academy of Hospice Physicians Palliative Care certification and exam within 2 years.
• Must have a Valid Florida drivers License, maintain current auto insurance coverage and have access to a reliable automobile.
• Ability to work unsupervised in the field, exercising discretion and independent judgment.
