Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health LPN to work with our Home Health team to deliver individualized care to patients in their homes in central Hillsborough county (South Tampa/Carrollwood area).

Locations: Home health visits in central Hillsborough county (including South Tampa, Carrollwood, West Tampa, Town 'N' Country, Westchase, and surrounding areas)
Schedule: PRN role; 10-15 home health visits per week. Required weekend rotations.
What you'll Do
- Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
- Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
- Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
- Perform patient and family instruction on diet, medication, and treatment.
- Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need
- Active Florida LPN license
- Applicants must be a graduate of an approved nursing program.
- Minimum of 1-2 years of experience as a LPN.
- Home health care experience preferred.
- Strongly prefers experience using Homecare Homebase (HCHB) EMR
- Intravenous (IV) Certification strongly preferred.
