Home Health LPN (PRN) - Empath Home Health - Venice, South Sarasota, Nokomis, Laurel, Siesta Key
Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health LPN to provide skilled nursing care for patients in their homes throughout the Venice, South Sarasota, Nokomis, Laurel, Siesta Key, and surrounding areas. In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.
Locations: Home health visits in the Venice, South Sarasota, Nokomis, Laurel, Siesta Key, and surrounding areas
Schedule: PRN; 10-15 home health visits available per week

What You’ll Do
- Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
- Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
- Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
- Performs patient and family instruction on diet, medication, and treatment.
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
What You’ll Need
- Active Florida LPN license
- Applicants must be a graduate of an approved nursing program.
- Minimum of 1-2 years of experience as a LPN.
- Home health care experience required.
- Strongly prefers experience using Homecare Homebase (HCHB) EMR
- Intravenous (IV) Certification preferred.
