Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits throughout Palm Beach County. This role offers the opportunity to support patients and families during critical moments by delivering compassionate, high-quality care in the comfort of their homes on the following schedule: 7am - 7:30pm and every other weekend.

What you'll Do
As a Continuous Care LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.
- Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
- Coordinate and provide individualized care plans that prioritize comfort and dignity
- Utilize monitoring, intervention, and teaching skills to enhance quality of life
- Provide emotional support and reassurance to patients and families during overnight care
- Ensure exceptional, personalized service and a positive experience for every patient and family

What You'll Need
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
