Role Description
This is a full-time on-site role for a Hospice Admissions Registered Nurse (RN). Based in Marion County FL, the role involves conducting patient assessments to determine hospice care eligibility and developing care plans tailored to patient needs. The RN will coordinate with interdisciplinary teams, communicate effectively with patients, families, and healthcare providers, and ensure compliance with hospice care standards and guidelines.
Qualifications
- Strong background in Medicine and Palliative Care
- Proficiency in Nursing practices, including patient assessments and care planning
- Experience in Home Care and Direct Patient Care settings
- Ability to work collaboratively in an interdisciplinary team environment
- Excellent communication and organizational skills
- Active and unencumbered Registered Nurse (RN) license in the state of Florida
- Previous hospice care experience is highly preferred
- Certification in hospice and palliative care nursing (CHPN) is a plus
