Emerest Home Care of Connecticut LLC, a large size home care agency, is looking for an experienced home health aide scheduler to join our Stratford-based home care team. This is a strictly in-office position for someone who has worked in the office of a licensed home care agency and understands the fast-paced scheduling environment, ideal for someone who’s organized, proactive, and mission-driven.

Responsibilities include but not limited to:
- Timely intake of client referrals in need of staffing
- Collaborate with Human Resource to staff and schedule clients in need of home care services.
- Maintain rapport and constant communication with referral sources, caregivers, clients and families.
- Oversee client care by conducting scheduled and unscheduled home and virtual visits with clients and caregivers.
- Knowledge of Sandata and Santrax Maintenance
1. Staffing and scheduling assigned caseload.
2. Communication with clients and families regarding set schedules and unscheduled events (no-shows)
3. Monitoring visits for no-shows
4. Communication with care managers regarding schedules and client updates.
5. Accepting new referrals from intake and staffing new referrals in a timely manner
6. Providing superior customer service to clients, caregivers, families, care managers etc.
7. Documenting all client and caregiver related activities
8. Maintaining client compliance
9. Constant customer support

Education and experience:
- At least one year of case coordination experience preferred.
Job Type: Full-time
Pay: $24.00 - $30.00 per hour
