Social Services Director Responsibilities:
- Lead and manage the Social Services department
- Support residents and families with emotional, social, and discharge planning needs
- Collaborate with the interdisciplinary team to ensure quality care and a smooth transition process
- Serve as a liaison for families, residents, and outside agencies
- Maintain accurate documentation in accordance with state and federal regulations
Social Services Director Qualifications:
- Active LSW license in Pennsylvania (required)
- Experience in long-term care or a similar healthcare setting
- Excellent communication, organizational, and leadership skills
- Strong knowledge of resident rights, care planning, and discharge processes
