What are the responsibilities and job description for the Hospice RN Admission Nurse position at Emerald Hospice?
We are looking for a compassionate, talented, and experienced Hospice Admission Nurse (RN) that is eager to join our Emerald Team.
We are seeking a compassionate and skilled nurse to provide high-quality, patient-centered care to individuals and families receiving hospice services. This role is essential in supporting patients through end-of-life care while ensuring dignity, comfort, and clear communication throughout the process.
Key Responsibilities:
- Provide education and guidance to patients and families regarding hospice services, philosophy, and end-of-life care
- Conduct initial patient and family meetings, including obtaining informed consents for hospice services
- Coordinate and oversee all aspects of new patient admissions
- Perform comprehensive head-to-toe physical assessments
- Develop and implement individualized care plans based on nursing assessments and diagnoses, under direction of hospice physician, focusing on immediate needs and goals
- Actively involve patients and families in the care planning process to ensure alignment with their preferences and values
- Educate families on patient condition, care needs, and expectations throughout the course of care
- Collaborate with interdisciplinary patient care teams to ensure seamless communication and continuity of care
- Communicate effectively with attending physicians, hospice team members, and external facilities to coordinate optimal care and appropriate use of resources
- Complete documentation in a timely and accurate manner in accordance with regulatory and organizational standards
- Participate in an assigned on-call rotation to ensure continuity of care
- Perform other duties as assigned
Job Requirements:
- Registered Nurse licensed in the state of Oklahoma,
- Valid Driver’s License and Automobile Insurance, Reliable Vehicle,
- Two (2) years of RN Case Management and/or Hospice Admission experience in a clinical care setting preferred; hospice experience of 1 year required
Pay negotiable, based on experience.
