Home Care Supervisor
REPORTS TO: Administrator/Assistant Administrator
JOB SUMMARY:
Under the supervision of the Administrator/Assistant Administrator the Home Care Supervisor selects, trains, evaluates, and supervises home care employees and provides in-service programs for home care staff. The Home Care Supervisor maintains the quality of home care and has complete knowledge of state licensure laws, certification requirements, and insurance qualifications for reimbursement. The Home Care Supervisor coordinates client services between Emerald Home Care and the community.
The Home Care Supervisor performs supervision contributing to the care of the clients and maintains professional standards. The Home Care Supervisor initiates and participates in the assessment of client needs.
DUTIES AND RESPONSIBILITIES:
· Assist with recruiting, interviewing, and selecting home care staff.
· Assist with orientation for new home care staff.
· Supervise and evaluate the performance of all home care staff.
· Recognize the physical, emotional, intellectual, and social needs of clients.
· Participate in on-call to ensure the delivery of quality client care services after hours.
· Supervise, direct, and assess client care and, when appropriate, provide direct client care.
· Understand the need to report changes in the consumer’s condition to physicians, consultations, and the Clinical Manager
· Instruct staff in the importance of teaching consumer self-care with ADLs.
· Interpret Emerald Home Care policies and procedures, state, and federal regulations for direct care personnel.
· Organize data in formats acceptable to physicians and insurance providers.
· Participate in case conferences with the employee and Administrator/Assistant Administrator as needed.
· Provide current information about home care services when requested by the Administrator/Assistant Administrator.
· Assist with quarterly review of home care records to ensure compliance with Emerald Home Care policies, state licensure laws, and certification requirements.
· Ensure ongoing review of all home care documentation.
· Maintain client confidentiality.
· Represent Emerald Home Care on selected home care or professional committees.
· Inform Emerald Home Care of ongoing educational opportunities in the field of home care.
· Respect consumer’s rights and ensure confidentiality.
· Review all referrals for placement.
· Accept cases that fit within Emerald Home Care’s model.
· Identify primary, secondary, other diagnoses and review plans of care for appropriateness of care. Assists in determining the frequency and length of visits or shifts required for a client and the level of staff person required.
· Assist with the development of care plans to ensure reimbursement.
· Assess clients and develop care plans.
· Communicates with MCO’s, Care Coordinators and other health professionals to plan and carry out the care plan.
· Modifies and carries out care plans based on ongoing assessment of client needs.
· Consults with physicians, health care related professionals, and family.
· Ensures and participates in supervision of all home care cases to evaluate staff, client care, and progress in accordance with regulatory requirements and Emerald Home Care policies.
· Reviews home care records to ensure appropriate documentation is current and pertinent to care prescribed.
· Assist in evaluating medical supply companies for product quality. Cost efficiency, and safety.
· Organize and teach courses and ensure competency evaluation for direct care workers and assist the Home Health line of service with Home Health Aides EVV, Scheduling and billing.
· Organize and assist in course set up with the Clinical Director/ Clinical Manager for the Home Health line of service on home care requiring Home Health Aides.
· Visit consumer care coordinators, discharge planners, social services, and community agencies with making presentations in the community to make Emerald Home Care programs more visible.
· Review the call dashboard daily on HHA Exchange for missed punches and verify that staff is in the right location.
· Maintain client documentation and charting for compliance.
· Maintain Service Authorization Forms (SAFs) for each consumer and update plans of care accordingly.
· Will direct the Home Care Coordinator and others in the home care department on projects that need completed and will follow up with staff to ensure all tasks are finalized in a timely manner.
· Assist the Administrator/Assistant Administrator and Home Health line of service with additional tasks and projects as directed.
QUALIFICATIONS/EDUCATIONAL REQUIREMENTS:
· College degree preferred from an appropriate institution.
· Highschool diploma or equivalent required.
· Minimum of three years’ experience within the home care setting, including teaching and supervisory experience.
Working experience in a high-pressure environment, including experience in adapting to changing priorities. Candidates must also have computer/ keyboard skills and proficiency in Microsoft Office software.
PHYSICAL DEMANDS:
· Lifting unspecified weights, depending on assignment requirements.
· Bending, scooping, stooping to prepare equipment, materials, or objects for treating clients or managing client care of the client environment.
· Office level work, Computer use, must be able to lift and/or move up to 50 pounds.
