What are the responsibilities and job description for the MSW/ Social Worker position at Emerald Health Services, Inc.?
MSW/ Social Worker Responsibilities:
- Intake screen (Check eligibility, fill out intake form, provide tour, etc.)
- Conduct initial assessments, bi-annual reassessments.
- Conduct initial home evaluations and home visits as needed.
- Individual and group counseling to elderly participants
- Connect participants with supporting resources, such as IHSS and Access Paratransit.
- Provide case management.
- Complete quarterly assessment, progress notes, flow sheets in a timely manner.
- Planning and coordination of discharges and manage discharge folders.
- Bilingual (spoken and/or written) in Chinese (Mandarin and/or Cantonese)
- Rotating Saturdays.
- H1b sponsorship available if qualify.
Job Type: Full-time
Pay: $25.00 - $30.00 per hour
