Duties
- Monitor patient condition throughout the night by observing vital signs and overall well-being using EMR (Electronic Medical Records) systems
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts
- Administer medications accurately following home health care regulations and medical documentation standards
- Provide specialized dementia and Alzheimer's care, ensuring safety protocols are strictly followed for memory care patients
- Support end-of-life care with compassion, dignity, and respect while maintaining patient confidentiality in accordance with HIPAA guidelines
- Perform catheter care, wound management, and basic first aid or CPR if necessary during emergencies
- Maintain detailed records of patient observations, medication administration, and medical documentation to ensure seamless communication with healthcare teams
Requirements
- Proven experience working with geriatric patients or in assisted living environments, nursing homes, or hospice care settings
- Strong knowledge of medical terminology, vital signs monitoring, medication administration, and patient personal care assistance
- Familiarity with EMR/EHR systems for accurate record-keeping and documentation
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts
- Excellent communication skills for effective client interaction and collaboration with healthcare professionals
- Valid certifications in CPR and basic life support (BLS), along with first aid training
- Experience working with individuals with disabilities or developmental disabilities is highly desirable
- Knowledge of home health regulations, hospice care practices, dementia/dementia care techniques, and geriatrics best practices
- Ability to work overnight shifts reliably while maintaining high standards of patient safety and comfort
